Wellness Visit Incentive
New employees must complete their bio-metric screening within 60 days after their effective date of insurance to qualify for the $25 per month premium savings.
Spouses of new employees or come on our medical plan during the plan year must complete their bio-metric screening within 60 days after their spouse's effective date to qualify for the $25 per month premium savings.
Tobacco Cessation Program
Employees and their dependents enrolled in one of the City's medical plans may participate the Tobacco Cessation Program through Wellness Works. The program covers 100% of prescribed nicotine replacement therapy for up to six (6) months.
A $25 monthly Tobacco Surcharge will be assessed for
- Any employee who does not use tobacco products & does not sign and turn in a Non-Tobacco User Affidavit by the established deadline.
- Any employee who uses tobacco products, but does not provide proof of participation in a qualifying Tobacco Cessation Program by the established deadline.
Spouses who are enrolled in the medical plan are not required to provide their tobacco use status, and are not subject to any Tobacco Surcharges.
