Medical Benefits Overview


Medical Insurance

Group medical insurance is provided to all regular, full-time employees.  No coverage is provided to regular, part-time or seasonal employees.

Employees may choose between three levels of medical coverage:

  • Health Savings Account Plan (HSA)
  • Health Reimbursement Account Plan (HRA)
  • Catastrophic Plan (CAT)

The Health Savings Account Plan (HSA) is part of the Open Access Plus (OAP) Network plans, which means that employees and their dependents have the option of seeking medical treatment through providers in the network or from providers outside the network.  The Health Reimbursement Account Plan (HRA) and Catastrophic Plan (CAT) are part of the Local Plus (LocalPlus) Network plans, which means that employees and their dependents have the option of seeking medical treatment through providers in the network or from providers outside the network. Services provided by In-Network providers will be covered at a higher level than those provided by Out-Of-Network Providers.

The waiting period for an eligible employee and their dependent(s), if elected, is 30 days from date of hire and coverage terminates the last date of the employee's employment with the City.  Employees may make changes to their coverage throughout the plan year if Human Resources is notified of any requested change in status within 30 days of the qualifying event.


Deductions for medical premiums are taken 24 times per year unless special circumstances apply.  Your election of medical benefits automatically enrolls you in the Pre-Tax Saver plan; this means that premiums for medical insurance are deducted from your gross income before taxes.  You must sign a waiver if you wish to opt out of the Pre-Tax Saver Plan.

Gap Insurance

Employees who elect the Catastrophic Plan are eligible to enroll in a Gap Insurance plan provided by Special Insurance Services, Inc. (SIS). View Gap Insurance Premiums & Plan Highlights.

  • The City will pay the premium for the Core Gap Plan for employee only.  The City will pay the employee only premium for the Buy-Up Plan if the employee completes the wellness incentive.

Survivor Continuation Coverage

In accordance with Article 6228f.1, Texas Revised Civil Statues, Rights of survivors of Certain Public Servants, the eligible survivor of a law enforcement officer or firefighter who suffers violent death in the course of performance of duty is entitled to purchase continued health insurance benefits.  An eligible survivor is defined under the Act as a surviving spouse or a dependent.  In order to receive continuation coverage, the City of Lewisville Human Resources Department must be informed no later than the 90th day after the date that the law enforcement officer or firefighter died that the survivor elects to continue coverage.  Additional information concerning survivor benefits, including eligibility requirements, may be obtained by contacting the Human Resources Department at 972.219.3489.
