YOU MAY OBTAIN A BIRTH/DEATH CERTIFICATE FROM THE CITY SECRETARY’S OFFICE IF:
- The person of record was born or died in the city limits of Lewisville
- You are a qualified applicant (the registrant; a member of his/her immediate family either by blood or marriage, including step-parent, step-sibling, etc.; his/her guardian; his/her legal representative or agent)
- The applicant is able to provide picture identification
REQUESTING A CERTIFIED BIRTH/DEATH RECORD
- download English application form
- download Spanish application form
- Return the application form by mail or in person
- Attach or bring in a picture form of identification
- Remit $23.00 for each certified copy of a birth record
- Remit $21.00 for the first and $4.00 for each additional certified copy of a death record
- May use overnight or express mail at the applicant’s expense
CREDIT/DEBIT CARDS ARE ACCEPTED FOR APPLICATIONS SUBMITTED IN PERSON; HOWEVER, PAYMENT FOR APPLICATIONS SUBMITTED BY MAIL MUST BE IN THE FORM OF A CHECK OR MONEY ORDER
