Medical Insurance


Group medical insurance is provided to all regular, full-time employees. No coverage is provided to regular, part-time or seasonal employees.

Employees have a choice of two levels of medical coverage – a Health Reimbursement Account Plan (HRA) and a Catastrophic Plan (CAT), which are Open Access Plus (OAP) Network plans. The premiums for employee coverage under the Health Reimbursement Account Plan are $21 per month; premiums for employee coverage under the Catastrophic Plan are free. Employees may elect dependent coverage and are responsible for the cost of dependent coverage. The waiting period for eligible employees and their dependents, if elected, are 30 days and coverage terminates the last date of the employee's employment with the City.

Under the OAP arrangement, employees and their dependents have the option of seeking medical treatment through providers in the network or from providers outside the network. Benefits for OAP provided services are higher than those provided out-of network. You may link to the Schedule of Benefits below.

Retiree Continuation Coverage

Employees who officially retire under TMRS after January 1, 1994 may continue health insurance benefits at retirement. The employee must elect to continue health coverage no later than the day on which the employee retires. The employee may elect to continue coverage for dependents covered under the health plan on the day the employee elects coverage continuation. A dependent not covered under the plan on the date the employee leaves municipal employment may not be covered under the plan. Spouse may be added with a change in family status as long as they had not previously termed from coverage on the retiree plan. If the retiree discontinues coverage after the initial election, the retiree is no longer eligible for coverage. Survivor Continuation Coverage

In accordance with Article 6228f.1, Texas Revised Civil Statutes, Rights of Survivors of Certain Public Servants, the eligible survivor of a law enforcement officer or firefighter who suffers violent death in the course of performance of duty is entitled to purchase continued health insurance benefits. An eligible survivor is defined under the Act as a surviving spouse or a dependent. In order to receive continuation coverage, the City of Lewisville Human Resources Department must be informed no later than the 90th day after the date that the law enforcement officer or firefighter died that the survivor elecst to continue coverage. Additional information concerning survivor benefits, including eligibility requirements, is provided separately by the Human Resources Department.

The City of Lewisville Health Benefit Trust is required by law to maintain the privacy of your protected health information and to provide you with notice of its legal duties and privacy practices with regard to your protected health information. As your group health plan, the City of Lewisville Health Benefit Trust must use and disclose protected health information in order to pay benefits to you and your health care providers. The City of Lewisville Health Benefit Trust uses physical, electronic, and procedural safeguards to protect your personal information from being used or disclosed inappropriately.
